Consent Form for Children in Virtual Cooking Classes

Parent or Guardian Consent: By allowing your child to participate in a virtual cooking class hosted by ThisIsChefT, you acknowledge that the class will involve preparing and tasting recipes and using common kitchen equipment, adapted for an online setting.

Class Design:

• Classes are tailored to suit the anticipated skill level of various age groups.
• Your child will engage in the following activities under the guidance of our culinary team via virtual instruction:
  • Ages 5-10: Mixing, measuring, stirring, light use of oven and stove (with adult supervision).
  • Ages 11-17: Cutting/chopping, using a stovetop and/or oven, operating electric equipment such as food processors, mixers, and blenders.

    Virtual Kitchen Rules:

    1. Safety First: Ensure a safe environment for your child to participate in the class, with adult supervision as needed. This includes maintaining a clear and organized cooking area.
    2. Hygiene and Dress Code: Proper clothing and shoes should be worn, and hair should be pulled back to maintain kitchen hygiene, even at home.
    3. Photography/Video Consent: Virtual classes may be recorded or photographed and used to showcase ThisIsChefT’s activities. Participation in the class includes consent to use these images and recordings.
    4. Allergy Awareness: It is the responsibility of the parent/guardian to inform the instructor of any food allergies or sensitivities your child may have.

    Disclaimer of Liability: ThisIsChefT is not responsible for any injuries or accidents that may occur during the virtual cooking class. This includes, but is not limited to, cuts, burns, or other kitchen-related injuries. It is the responsibility of the parent or guardian to supervise and ensure the child’s safety throughout the class.

    By consenting to your child’s participation in ThisIsChefT’s virtual cooking classes, you agree to adhere to these guidelines and accept responsibility for your child’s safety and wellbeing during the class.

    Cancellation and Refund Policy: All sales for ThisIsChefT’s virtual cooking classes are final. No cancellations or refunds are permitted once a booking is confirmed.

    By consenting to your child’s participation in ThisIsChefT’s virtual cooking classes, you agree to adhere to these guidelines and accept responsibility for your child’s safety and wellbeing during the class. You also acknowledge and agree to the cancellation and refund policy stated above.
